Nomination Form Instructions

Eligibility

1. - Before filling out the form, please ensure that you:
2. - Are a bonafide PHAB member in good standing (membership fees and information updated).
3. - Meet the eligibility criteria set by the Election Commission for the position you intend to contest.
4. - Are not holding any conflicting office as per the PHAB bylaws.

Section 1 – Candidate Information

1. - Write your full name in English (in capital letters) and in Bangla.
2. - Provide your father’s/mother’s name, date of birth, and NID number.
3. - Indicate your membership type and voter ID (e.g., LM-120 or M-120).
4. - Provide your present address, email, and contact number.
5. - Clearly specify only one position you wish to contest for. (Multiple nominations are not allowed.)

Section 2 – Proposed and Seconded by

1. - Obtain endorsement from two PHAB members in good standing — one as Proposer and one as Seconder.
2. - Each must clearly write their name, membership type, voter ID, you will print the PDF version at the end and collect signature, and date in the form physically
3. - Ensure both -Proposer and Seconder are listed in the official PHAB voter list.
4. - Section 3 – Candidate’s Pledge & Agreement
5. - Read the pledge carefully before signing.

Write your full name and date to confirm that the information provided is correct and that you agree to abide by PHAB’s election rules and code of conduct. Then you progress for submission, it will allow you to double check information of your form and then print it for submission with required signatures.
